Heart Failure Disease Management Program

The purpose of this workshop is to provide attendees with evidence-based information on the essentials of heart failure disease management programs (HFDMP), including space and personnel requirements, creating a budget, marketing ideas, developing an inpatient program, transitioning patients, tools and innovative ideas for managing systems, models of care, and measuring outcomes.

All attendees will receive tools and information necessary for implementing a heart failure program at a local level or for enhancing an existing heart failure program.

Educational content will be presented by heart failure nurse and physician experts, as well as a billing/coding expert. The presenters will share their knowledge, advice and experience with participants. Time for questions and answers will be provided for interaction between the presenters and the audience.

Objective

At the end of the program, the participant will be able to:



  • Identify the necessary equipment and system processes required for starting a heart failure program
  • Discuss three elements that should be included in a heart failure program budget, and describe opportunities to maximize billing resources
  • Outline ideas for an inpatient heart failure disease management program
  • Describe at least 3 models of care for heart failure disease management
  • Discuss financial and clinical outcome measures for a heart failure program.
